Subject: Sick Leave for Dental Appointment
Respected Sir,
I’m writing to let you know that I, Daniyal Irfan, Senior Manager at Raj Fabrics, have been suffering from tooth pain since last week. My teeth have been bothering me for some months, but they have just begun to hurt severely, and the pain will not go away no matter what medication I take. I consulted a local doctor a few weeks ago, who offered various medications and natural treatments for temporary comfort, but they weren’t much assistance. I went to the dentist over a week ago because the pain was terrible. He informed me that I had an ingrown tooth and that if I do not get it pulled soon, it will become permanent and may cause a gum infection. The doctor has scheduled an appointment for Sunday, September 12th, and has advised me not to leave the house for at least a week following the extraction. This little operation is set for Sunday, and it will take a week for my gums to recover. After the operation, I’ll be on a lot of medicine, and these pills might make you dizzy. I’m already going to be a week post-surgery since I’ve been told to drink just liquids and won’t be able to eat solid meals for a week.
I won’t be able to focus properly on my work this way, and I don’t want my health to take precedence over my career in any manner. This is why I’d like to request a one-week sick leave so that I may focus entirely on my health and recover rapidly so that I can return to the workplace as soon as possible. I will be eternally thankful to you for your wonderful gesture.
Yours Sincerely,
Daniyal Irfan

Subject: Sick Leave for Dental Appointment
Respected Sir,
This is to bring to your kind notice that I, Ashok Khundra, Senior Manager at your company, For the past four days, I’ve been suffering from a terrible toothache. I went to the dentist about it, and he gave me some antibiotics to help me heal quickly, but it hasn’t been much of a help. I tried numerous home cures and other medications, but none of them helped me ease my discomfort. I’m in a lot of pain and suffering, and it’s affecting how I sleep, eat, and go about my regular routines. It is nearly difficult for me to chew anything, and even drinking water is excruciatingly painful. It’s possible that I have gum inflammation or an ingrown tooth that’s giving me a lot of discomforts. I’ve recently seen a new dentist, who advises me to have a small surgery as soon as possible, otherwise, the problem will worsen over time. This problem has also had an impact on my office work since I am unable to sleep adequately, resulting in a lack of concentration on the job. I can’t seem to concentrate on my job because of the discomfort produced by my toothache. This has an impact on my everyday performance, and it can cause me a great lot of shame when the employee assessment is held.
I don’t want my health to overshadow my work in any possible way so I would like you to grant me sick leave for three days so I have enough time to get my surgery done and recover as soon as possible. I’ll be extremely grateful to you for this act of kindness.
Yours Sincerely,
Ashok Kundra

Subject: Sick Leave for Dental Treatment
Respected Sir,
With great respect and honor, I would like to declare that I, John Hardik, Senior Manager at Tellus Communications, am suffering from excruciating pain in my tooth and there is no other option than to have it out. Removing means I’ll have to have my tooth extracted, which means I’ll need to take a day off from work. I had scheduled my visit with the dentist based on preoperative evaluation criteria. The dentist will then advise me on what should be done to the tooth and when it should be done. I have no idea how long this operation will take, nor when I will be in perfect condition.I made an appointment with one of the top dentists in town to obtain these answers, and I have to go there the day after tomorrow for my dental evaluation.
I don’t want my health to interfere with my work in any way; I’ve always been the best at this business and want to keep that title for a long time, therefore I’d want you to allow me sick leave for the day after tomorrow so I may get my tooth checked by the dentist and recover as soon as possible. I will be eternally thankful to you for your wonderful gesture.
Yours Sincerely,
John Hardik
