I hope you will understand my health condition and I want to request you for leave of one week from 21st September to 27th September, 2015. I will try to join back as soon as I feel my perfect health condition. I hope you will sanction me the leave and I assure you that I will cover up the official tasks side by side and will be in touch with my assistant so that it may not cause any inconvenience in official working as well. I would be highly obliged for this.

Respected Sir,

This is to inform you that unfortunately I am in bad state of health. The dismaying thing is, I am suffering from Asthma Attacks from last month. Doctor has advised me to stay at home and avoid external exposure and travelling. For this, I will be requiring leave for few days i.e 3rd June to 7th June, 2015. I really hope that I would be fine within these days and come back to office. In order to avoid any hurdle in work I shall be in touch with my team through email. I hope you will consider my request.

With due respect and reverence, it is stated that I cannot come to office today due to my condition. I have asthma and it gets really irritating sometime. Asthma is a chronic lung disease that inflames and narrows the space. Due to exposure to irritants and contaminated pollutants, my asthma gets really bad sometimes.
Asthma triggers vary from person to person. My asthma trigger is airborne substances such as pollen, mites, molds and spores. Due to this condition, I have constant coughing from night to day and vice versa. It would be really kind of you to give me leave for one day only. Thank you.
