He prescribed an antibiotic course for me to take for over a week and recommended that I stay at home until I recovered.

I sincerely request that you, the supervisor of my firm, accept my apologies and grant me sick leave. I don’t want my poor health to impact the way I work. Thank you so much for your time and generosity.
